Company Rehearsal Absence Request Form

Company Dancers Only

Use this form to request an absence from a Company Rehearsal. Must be requested 30 days in advance.

Medical Hold Form

If your dancer becomes injured please fill this out in order to put their account on pause. 

Drop Class Form

If you have to make an adjustment to your schedule, please fill this form out and submit before the end of the month.
